Please bring it with you to your 1st appointment. We recommend the following:

Arrive 15 minutes early to allow for processing

Bring completed forms with you to the appointment

Bring all of your insurance cards and a photo ID

Bring the CD with your MRI (and the report) with you (Exception: Mt Auburn & Shields MRI)

Insurance and Payment Policies
Payment Policy
Due to recent changes in healthcare plans, you may have questions regarding insurance or patient responsibility for services rendered. Our rates are representative of the usual and customary charges for our northeast area. Please review our payment policy information below. Copayments, deductibles, and self-pay are due at the time of the visit and will be charged to a credit or debit card.

Insurance
We participate in Medicare and most major commercial plans. If we accept your plan, please bring your insurance card with you for coverage verification. It is your responsibility to know your insurance benefits, your co-pays, and deductibles. Please contact your insurance company with any questions you may have regarding your coverage. We are happy to provide you with the diagnosis code and the procedural code, so you could inquire about your coverage and your responsible portion of the bill (when applicable).
Self-Pay
Some patients may choose not to use insurance for medical or procedural visits. In some cases, certain procedures are not covered by the insurance, and you may choose to self-pay for such treatments. We will be transparent and provide you with an estimate of the cost of service. All payments are due at the time of the visit and will be charged to a credit or debit card.

Co-payments and Deductibles
All co-payments and deductibles must be paid at the time of service. This arrangement is part of your contract with your insurance company. Failure on our part to collect co-payments and deductibles from patients can be considered fraud. Please help us in upholding the law by paying your co-payment and/or deductible at each visit. Payments can be made in the form of cash, personal check, or credit card. We accept MasterCard, Visa, Discover.

Coverage Changes
When your insurance changes, your coverage will change as well. For new or established patients, we need to have the information about your new insurance prior to your appointment. Please call us at 617-547-7163 to update us and bring the new insurance card to your upcoming appointment. You are responsible for the balance of your payment if your insurance company does not cover the charges within 30- 60 days of the visit.
